All applications and renewal packets must be completed electronically and emailed to [email protected] along with a copy of the payment receipt, and social security card.
Name on social security card and drivers license must match
Clear copies of SS card and license are requested, state will deny and close the case if they cannot make out the letters/ numbers.
ALL pages of the application must be completed by employee (In-state verification included).
The only place to leave blank is where it asks for name and signature of employing district
